After the license application is submitted you may receive a letter from AHCA with comments that says AHCA has conducted the initial review of the application and found deficiencies. You will have 21 days to respond.

Change of ownership occurs when the licensee sells or transfers its ownership to an individual or entity(as defined) as evidenced by a change in FEIN or TIN, or an event in which 51% or more of the ownership, shares, membership, or controlling interest of a licensee is transferred or assigned. All required forms must be received by AHCA at least 60 days prior to the date of change of ownership. AHCA requires the new owner to complete the Proof of Financial Ability to Operate.
