# SERVICES AND PRICES  FOR THE PROOF OF FINANCIAL ABILITY TO OPERATE

## Initial Application

#### $3,000-Nurse Registry 

##### Some key features

- Projections of one year 
- Six schedules included
- Preparation service
- Footnotes prepared in accordance with Generally Accepted Accounting Principles
  
     
  
  The  $3,000 price may vary based on size, long-term leases and complexity. If the lease is under 12 months then there is no extra charge, if the lease is more than 12 months then there is a $750 charge. There is an additional $1,000 charge for preparing a response to an Omission Notice.

## Initial Application

#### $4,000-5,000-Other

- Projections for two years
- Seven schedules included
- Compilation service
- Footnotes prepared in accordance with Generally Accepted Accounting Principles
  
  The  price may vary based on complexity ( i.e. additional services Franchise, Medicare, Medicaid , and  for each long-term lease.  Each additional service is $1000. There is an additional $1,000 charge for preparing a response to an Omission Notice.

- Same services as  initial application  plus more 
- Review of transaction documents to acquire license and review of prior year tax returns of entity to be acquired.
- Set up of acquired assets and debt and/or equity used to acquire license.

## Consulting Services

#### We Can be a Resource to You

We offer consulting services for those who do not need a CPA to prepare to  compile your Proof of Financial Ability to Operate  form. 

- Help with the form
- Understanding  and following the instructions
- Understanding and applying generally accepted accounting principles and footnotes
- Assist  your accountant or bookkeeper if they are unfamiliar with the AHCA form

##### Healthcare Clinic

Providers at primary care clinics are often general practitioners who provide a broad level of healthcare.

##### Home Health Agency

Primarily engaged in providing skilled nursing services and other therapeutic services.

##### Home Medical Equipment

Sellers or renters to consumers of home medical equipment includes delivery, set up and maintenance.

##### Nurse Registry

Contracts for Rn's, LPN's, CNA's, HHA,s companions, or homemakers, who are paid as independent contractors.

##### Mental Health Services

Helping patients with mental health conditions, and staffed by psychologists and social workers.

##### Homemaker/Companion Services

Providers of housekeeping, meal planning and prep, shopping assistance, and routine household activities.
